Considering everything, we have been pretty lucky so far and not uncovered any real issues that have impacted our designs or the budget.  That was until today.  And in the grand plans, it wasn’t all that bad.  Prior to starting work we didn’t know exactly how certain parts of the upstairs landing and kitchen ceiling/roof supports were actually done.   Basically they weren’t done all that well.  No where near to code that is for sure.  As a result and to meet code our kitchen ceilings have to be a few inches lower than the rest of the house.  Initially we knew this was coming and thought it would be about 4 inches lower, not to bad.  But today at one point they only plan they came up with that didn’t require a change order was going to lower the ceiling 10 inches, not 4.  Which meant out would go the coffered ceiling and our cabinets would now be a lot closer to the ceiling then designed for.  So after 30 minutes of back and forth everyone came up with a new plan that provides the support the upstairs landing area needs but still only requires the ceiling to be lowered by 4 inches or so.  The good news is the change should be under $1K.  Mostly extra material and some labor.
